Health Services

 STATE REQUIREMENTS: 

In order to be in compliance with Pennsylvania Department of Health Regulations, all students must be immunized or have documentation regarding medical or religious exemption. Immunizations should be completed on the first day of school per State Law.

  • · 4 doses of tetanus* (1 dose on or after the 4th birthday)
  • · 4 doses of diphtheria* (1 dose on or after the 4th birthday)
  • · 4 doses of Polio (4th dose on or after 4th birthday and at least 6 months after previous dose)
  • · 2 doses of measles**
  • · 2 dose of mumps**
  • · 1 dose of rubella (German measles)**
  • · 3 doses of hepatitis B
  • · 2 dose of varicella (chickenpox) vaccine or history of disease
  • * Usually given as DTP or DTaP or DT or Td
  • ** Usually given as MMR
Emergency Procedure Form. This is a very important part of your child’s record. The front of the form authorizes the school district, and emergency personnel if necessary, to administer medical treatment during school hours. It is important that you include more than one emergency contact so that your child can be treated promptly. If any of this information changes during the school year, please let us know.
The back of the form is your child’s health history, and becomes part of their permanent record. We ask that you update your child’s history each year so that we can better care for you child. Please make sure that you complete both sides, sign and date the form, and return it as soon as possible.

Additional Information: Please encourage your child to eat a healthy breakfast each morning. Please be sure that your child is dressed appropriately for the weather conditions. Some clothing is available for children who become soiled during the school day. Please launder and return any borrowed clothing the following day (or upon returning to school).
If your child has an injury or illness that requires a limitation of their recess and/or physical education activities, please send a note to the teacher or nurse, so we can help with this request. Durable Equipment and Elevator Use: A Physician’s note should be provided if a student will require the use of crutches and/or an elevator for more than one day.

Please note that durable equipment such as crutches and wheelchairs must be obtained by the family so that school equipment is available for emergencies. Healthy children learn best. If you have any health concerns regarding your student, please do not hesitate to contact your school nurse. Together we can do more for our children than any of us can accomplish alone.

Required Exams

  • Physical K/1, 6th, 11th Grades or upon entry
  • Dental Exam
  • K/1, 3rd, 7th grade or upon entry

Yearly Health Screenings

  • Height and Weight will be completed on all students Grade K-12.
  • Vision screening will be completed on all students Grade K-12.
  • Hearing Screening will be completed on students in K-3, 7 and 11.
  • Scoliosis Screening will be completed on students in 6th and 7th grade.
